11. Maybe there is something toxic where you live that is causing you

to have this reoccur. I had the same problem when I lived in Texas. It turns out I had severe allergies to the Juniper trees and mold. The solution was to leave Texas or load up on the steroids and antibiotics that weren't doing much good. So my DH and I packed up and left Texas. Now I know you said you have been tested for allergies, but the tests are for the known allergens in your area and may not target something that may be emitting from your house or even pollution that is being covered up by industry in your area. Think Love Canal.

Take close inventory of your environment, I think you will find a cause there and then it will be easier to find a targeted solution. After leaving Texas I still had residual bronchitis and found that large doses of echinacea helped as well as honey. I still eat a tspn of honey a day every day. Now I am hesitant to tell you what worked for me because it crosses into the border of giving medical advice, so instead seek out a medical professional, who has a strong background in herbal remedies before you try this. But first find out what is causing it.
