The card attached to the brochure were in my strongbox along with passport, birth certificate, etc. It has six pages of information illustrated with cartoons of a John Q. Public type going through all of the steps to assure proper coverage.

On the last page, in little print on the bottom it says:

U.S. Printing Office 1954
