At least citizens, and green card holders. Tourists can be covered by some kind of entrance fee. Hit anyone who employs undocumented migrants in a bid to evade OSHA etc.
Everyone needs to be covered all the time to move more care to business hours and to get savings from preventative care plus faster treatments before things get serious. Every actuarial estimate forecasts immediate savings that will grow after the transition period. The secondary effects like cheaper car insurance, lower workmans comp will help most people. I'm dreaming of some overnight ER shifts where they have to play cards or watch TV while on the clock in balance against the occasional busy times.
