Hear me out: one of the reasons that we are not supposed to make diagnoses over television or smart phone is it the patient presentation is very important in assessing their issues. Experienced clinicians can sense when there is an overriding condition which is greater than the sum of the symptoms… It’s sort of like gestalt medicine. In hospitals in the old days, they used to have a term FLK which was an abbreviation for funny looking kid. This was not to disparage the child, but to alert other practitioners that something was wrong, we just didn’t know what it was at the time. That has been supplanted by the Abbreviation UAC for unusual appearing child. It indicates that testing needs to be done to determine the exact problem or syndrome which is attached to the child. So I coined the term to myself: FLP for funny, looking president and then thought about things

One of the things that I realized I was not doing when observing Trump was thinking outside the box. His combination of symptoms, which includes loss of verbal inhibitions, the obvious placement of a Foley catheter, the droopy lip, the puffiness in the face, leads me to believe that he may have a brain tumor. I don’t think he has Lyme disease because he would never be bitten by a deer tick given his lifestyle, but it occurred to me that the sum of his symptoms is much greater than simplicity of analysis might explain.

I’m going to go a step further and say that it may be undiagnosed… That he is so bombastic and will fall that no one would dare suggest this out loud because the implications. If I were his physician, given his symptoms, I would order a test to rule out Brain tumor as a causation.

Another disorder he might be having is Guillain-Barre syndrome which does manifest itself with loss of bladder control and muscle weakness beginning with leg weakness. It would explain the ecchymosis in his hands because they receive IV immunoglobulin. But it doesn’t explain his significant cognitive decline, especially with his clear loss of inhibition. I had a patient who was a very famous judge in Philadelphia, who I knew was having a disorder early on because his speech pattern changed, and his use of profanity increased dramatically in inappropriate times. Just saying…

Because as “whatever” that he has ever been, something has significantly changed, and I’m not sure that he had a stroke which has been posited here and elsewhere. Quite frankly, he is not exhibiting signs of a stroke, more like encroaching, slow brain dysfunction. Of course, this is not a diagnosis, it’s a supposition, and in my opinion, needs to be ruled out by physicians if one cared about the health of the individual. I’m more concerned about the health of the nation, but what do I know… I only work here.
